NOTE: Due to the abuse of the Personal Use confirmation form by some of our users, it has been disabled.
The forms on both pages are identical, the difference is that the requests for confirmations to meet legal requirements are manually reviewed, edited and confirmed by our volunteers, while the confirmations for your personal use only are not.
